ABSTRACT

This study aimed to identify the phylogenetic similarities among the muntjac (Muntiacus spp.). The phylogenetic similarities among seven major muntjac species were studied by comparing the nucleotide sequence of 16s rRNA and cytochrome b genome. Nucleotide sequences, retrieved from NCBI databases were aligned by using DNASTAR software. A phylogenetic tree was created for the selected species of muntjac by using the maximum likelihood method on MEGA7 software. The results of nucleotide sequences (16s rRNA) showed phylogenetic similarities between, the M. truongsonensis and M. rooseveltorum had the highest (99.2%) while the lowest similarities (96.8%) found between M. crinifrons and M. putaoensi. While the results of nucleotide sequences (Cty b) showed the highest similarity (100%) between M. muntjak and M. truongsonensis and the lowest s (91.5%) among M. putaoensis and M. crinifrons. The phylogenetic tree of muntjac species (16s rRNA gene) shows the main two clusters, the one including M. putaoensis, M. truongsonensis, M. rooseveltorum, and M. muntjak, and the second one including M. crinifrons and M. vuquangensis. The M. reevesi exists separately in the phylogenetic tree. The phylogenetic tree of muntjac species using cytochrome b genes shows that the M. muntjak and M. truongsonensis are clustered in the same group.


Este estudo visou identificar as semelhanças filogenéticas entre os muntjac (Muntiacus spp.). As semelhanças filogenéticas entre sete grandes espécies muntjac foram estudadas comparando a sequência de nucleótidos de 16s rRNA e genoma citocromo b. As sequências de nucleótidos, obtidas a partir de bases de dados NCBI, foram alinhadas utilizando o software DNASTAR. Foi criada uma árvore filogenética para as espécies selecionadas de muntjac utilizando o método de probabilidade máxima no software MEGA7. ABSTRACT

Indian crested porcupine is the largest rodent pest that damages a wide variety of crops, vegetables, and tree species which ultimately causes huge economic loss in Pakistan, which is an agricultural country. It prefers to live in hilly terrain but common in temperate and tropical forests, shrublands, and grasslands. This study focused on the identification and assessment of crops damaged along with the main precautionary measures used by the local farmers. The data was collected from twenty-four villages of two union councils i.e. Chamhad and Slahad of district Abbottabad. Two types of data (primary and secondary) were collected from the study area. Primary data was collected for identification and estimation calculation of total crop damaged through direct field observation by taking random quadrates in each village of the study area. The damage in the crop was assessed by randomly selecting a quadrate of 1x1 m2 for the wheat, pearl millet, and Sorghum fields. While 4x4 m2 quadrates were taken for maize and vegetables. At least three quadrate samples were taken from each field including one quadrate taken from the center of the field area. In union council Chamhad, damage to maize (11.31%) and wheat (0.73%) by the Indian crested porcupine while in union council Salhad, damage of maize (6.95%) and wheat (1.6%) was observed. In the entire study area, overall damage to maize crop (8.01%) and wheat (0.88%) was calculated. Based on information obtained from the farmers, the Indian porcupine inflicted damage to potato, tomato, cauliflower, chili pepper, turnip, radish, pea, and onion, etc. Secondary data obtained through a questionnaire survey to explore the human porcupine conflict and precautionary measures used by the farmers and landowners. Open and closeended questionnaires (159) highlighted the presence of Indian crested porcupine in the study area and 96% of the respondents have seen porcupine directly. Many types of precautionary measures were used by the farmers such as fencing, night stay, night firing, and dogs to decrease the crop damage, respondents (63.91%) use guns for hunting. however, due to the largely agricultural area and nocturnal behavior of Indian crested porcupine majority of the respondents (51.57%) did not use any precautionary measure. Biological control of Indian porcupine is recommended in the study area. Farmers should be encouraged and provide incentives and killing through current should be banned while proper hunting license should be issued to overcome overhunting. Scientific studies are required to control the reproduction of porcupine specifically in the more damaged areas.


O porco-espinho indiano é a maior praga de roedores que danifica uma grande variedade de culturas, vegetais e espécies de árvores, o que acaba por causar enormes perdas econômicas no Paquistão, que é um país agrícola. Prefere viver em terrenos montanhosos, mas comuns em florestas temperadas e tropicais, arbustos e pastagens. Este estudo concentrou-se na identificação e avaliação das lavouras danificadas, juntamente com as principais medidas de precaução utilizadas pelos agricultores locais. Os dados foram coletados de 24 aldeias de dois conselhos sindicais, ou seja, Chamhad e Slahad, do distrito de Abbottabad. Dois tipos de dados (primário e secundário) foram coletados da área de estudo. Foram coletados dados primários para identificação e cálculo de estimativa do total da cultura danificada por meio da observação direta do campo, tomando quadrantes aleatórios em cada aldeia da área de estudo. O dano na cultura foi avaliado pela seleção aleatória de um quadrante de 1x1 m2 para os campos de trigo, milheto pérola e sorgo, enquanto quadrantes de 4x4 m2 foram tomados para milho e legumes. Pelo menos três amostras de quadrante foram colhidas de cada campo, incluindo um quadrante retirado do centro da área do campo. No Conselho Sindical de Chamhad, houve danos ao milho (11,31%) e ao trigo (0,73%) pelo porco-espinho indiano, enquanto no Conselho Sindical de Salhad, danos ao milho (6,95%) e ao trigo (1,6%) foram observados. Em toda a área do estudo, danos gerais à cultura do milho (8,01%) e ao trigo (0,88%) foram calculados. Com base em informações obtidas dos agricultores, o porco-espinho indiano infligiu danos à batata, tomate, couve-flor, pimenta, nabo, rabanete, ervilha, cebola, etc. Dados secundários foram obtidos por meio de um questionário para explorar o conflito suíno humano e as medidas de precaução utilizadas pelos agricultores e proprietários de terras. Questionários abertos e fechados (159) destacaram a presença de porco-espinho indiano na área de estudo e 96% dos entrevistados viram o porco-espinho diretamente. Muitos tipos de medidas de precaução foram utilizados pelos agricultores, como esgrima, estadia noturna, fogo noturno e cães para diminuir os danos na lavoura. Dos entrevistados, 63,91% usaram armas para caçar. No entanto, devido à área ser em grande parte agrícola e ao comportamento noturno da maioria dos porcos-espinhos indianos, 51,57% não usaram nenhuma medida de precaução. O controle biológico do porco-espinho indiano é recomendado na área de estudo. Os agricultores devem ser encorajados e fornecer incentivos, e o uso da corrente para matar os animais deve ser banido, assim como deve ser emitida a licença de caça adequada para superar a caça excessiva. Estudos científicos são necessários para controlar a reprodução de porco-espinho, especificamente nas áreas mais danificadas.

Microhabitat factors associated with the habitat of barking deer (Muntiacus vaginalis) were examined and compared between summer and winter seasons. Habitat characteristics and preferred habitat were measured by locating direct and indirect signs. To quantify the habitat utilization of barking deer, each selected study site was sampled for floral diversity from 2015 to 2017. Quadrats were deployed along transect lines to determine seasonal distribution. Barking deer were not evenly distributed across vegetation types in the study area; they occurred more often in the broad-leaved forest than in Chir pine forest, at an elevational range of 550-850 m, in thick vegetation on steep slopes. The most preferred habitat included trees and shrubs with 30% and 69% cover, respectively. Barking deer avoided thicker tree cover, possibly as it hinders movement and escape from predators. No significant difference (χ2 = 6.37, df = 3, p = 0.19) in seasonal vegetation cover was recorded.


Fatores de micro-hábitat associados ao hábitat do veado (Muntiacus vaginalis) foram examinados e comparados entre as estações de verão e inverno. As características do hábitat e o hábitat preferido foram medidos, localizando sinais diretos e indiretos. Para quantificar a utilização do hábitat de cervos-latidos, cada local de estudo selecionado foi amostrado para a diversidade floral de 2015 a 2017. Quadrats foram implantados ao longo de linhas de transecto para determinar a distribuição sazonal., Veados-latidos não foram distribuídos uniformemente pelos tipos de vegetação na área de estudo; ocorreram com mais frequência na floresta de folhas largas do que na floresta de pinheiros Chir, em uma faixa de elevação de 550-850 m, em vegetação densa, em encostas íngremes. O hábitat mais preferido incluía árvores e arbustos com 30% e 69% de cobertura, respectivamente. O veado-latido evitou uma cobertura de árvores mais espessa, possivelmente porque impede o movimento e a fuga de predadores. ABSTRACT

Cranes are the large and attractive Creatures of nature with long necks, legs, and life-span. Adults of both sexes are the same with similar color patterns. Demoiselle cranes spend most of their lifespan on dry grasses. They are also found around the stream, rivers, shallow lakes, natural wetlands, and depressions. To evaluate the current status of habitat use and major threats a study was conducted in tehsil Domel district Bannu. Line transect method and water quality tests (temperature, PH, contamination of E-coli bacteria) were used. To determine the major threats questionnaire method was used. The whole data was analyzed by using SPSS 21 version. Based on the distribution four study sites were selected and four water samples from each study site were taken. Most sites were moderate to highly degraded except Kashoo and kurram river mixing point which was low degraded with livestock grazing and human activities. Water quality tests showed PH ranges from 7-9, temperature 6.5-8.5, and contamination of E-coli in all samples. The social survey revealed that hunting, habitat degradation, and pollution as major threats. Effective long-term conservation and management in the study area are needed to focus on the protection of disturbance-free habitat.


Guindastes são a grande e atraente criatura da natureza com um pescoço longo, pernas e vida útil. Adultos de ambos os sexos são os mesmos com padrões de cores semelhantes. Guindastes Demoiselle passam a maior parte da vida em gramíneas secas. Também se encontram ao redor do córrego, rios, lagos rasos, pântanos naturais e depressões. Para avaliar o estado atual do uso do hábitat e as principais ameaças, um estudo foi realizado no distrito de Tehsil Domel, em Bannu. Foram utilizados o método transect e testes de qualidade da água (temperatura, pH, contaminação da bactéria E. coli). Para determinar as principais ameaçasfoi utilizado o método de questionário. Todos os dados foram analisados por meio da versão SPSS 21. Com base na distribuição, foram selecionados quatro locais de estudo e quatro amostras de água de cada local de estudo. A maioria dos locais estava moderada a altamente degradada, exceto no ponto de encontro dos rios Kashoo e Kurram, que teve baixa degradação com pastagem de gado e atividades humanas. Os testes de qualidade da água mostraram variação de pH de 7-9, temperatura 6,5-8,5 e contaminação de E. coli em todas as amostras. A pesquisa do questionário revelou que a caça, a degradação do hábitat e a poluição são as principais ameaças. Efetiva conservação e gestão a longo prazo na área de estudo são necessárias para se concentrar na proteção de um hábitat livre de distúrbios.

Quadrats were deployed along transect lines to determine seasonal distribution. Barking deer were not evenly distributed across vegetation types in the study area; they occurred more often in the broad-leaved forest than in Chir pine forest, at an elevational range of 550-850 m, in thick vegetation on steep slopes. The most preferred habitat included trees and shrubs with 30% and 69% cover, respectively. Barking deer avoided thicker tree cover, possibly as it hinders movement and escape from predators. No significant difference (2 = 6.37, df = 3, p = 0.19) in seasonal vegetation cover was recorded.


RESUMO Fatores de micro-hábitat associados ao hábitat do veado (Muntiacus vaginalis) foram examinados e comparados entre as estações de verão e inverno. As características do hábitat e o hábitat preferido foram medidos, localizando sinais diretos e indiretos. Para quantificar a utilização do hábitat de cervos-latidos, cada local de estudo selecionado foi amostrado para a diversidade floral de 2015 a 2017. Quadrats foram implantados ao longo de linhas de transecto para determinar a distribuição sazonal., Veados-latidos não foram distribuídos uniformemente pelos tipos de vegetação na área de estudo; ocorreram com mais frequência na floresta de folhas largas do que na floresta de pinheiros Chir, em uma faixa de elevação de 550-850 m, em vegetação densa, em encostas íngremes. ABSTRACT

Abstract Indian crested porcupine is the largest rodent pest that damages a wide variety of crops, vegetables, and tree species which ultimately causes huge economic loss in Pakistan, which is an agricultural country. It prefers to live in hilly terrain but common in temperate and tropical forests, shrublands, and grasslands. This study focused on the identification and assessment of crops damaged along with the main precautionary measures used by the local farmers. The data was collected from twenty-four villages of two union councils i.e. Chamhad and Slahad of district Abbottabad. Two types of data (primary and secondary) were collected from the study area. Primary data was collected for identification and estimation calculation of total crop damaged through direct field observation by taking random quadrates in each village of the study area. The damage in the crop was assessed by randomly selecting a quadrate of 1x1 m2 for the wheat, pearl millet, and Sorghum fields. While 4x4 m2 quadrates were taken for maize and vegetables. At least three quadrate samples were taken from each field including one quadrate taken from the center of the field area. In union council Chamhad, damage to maize (11.31%) and wheat (0.73%) by the Indian crested porcupine while in union council Salhad, damage of maize (6.95%) and wheat (1.6%) was observed. In the entire study area, overall damage to maize crop (8.01%) and wheat (0.88%) was calculated. Based on information obtained from the farmers, the Indian porcupine inflicted damage to potato, tomato, cauliflower, chili pepper, turnip, radish, pea, and onion, etc. Secondary data obtained through a questionnaire survey to explore the human porcupine conflict and precautionary measures used by the farmers and landowners. Open and close-ended questionnaires (159) highlighted the presence of Indian crested porcupine in the study area and 96% of the respondents have seen porcupine directly. Many types of precautionary measures were used by the farmers such as fencing, night stay, night firing, and dogs to decrease the crop damage, respondents (63.91%) use guns for hunting. however, due to the largely agricultural area and nocturnal behavior of Indian crested porcupine majority of the respondents (51.57%) did not use any precautionary measure. Biological control of Indian porcupine is recommended in the study area. Farmers should be encouraged and provide incentives and killing through current should be banned while proper hunting license should be issued to overcome overhunting. Scientific studies are required to control the reproduction of porcupine specifically in the more damaged areas.


Resumo O porco-espinho indiano é a maior praga de roedores que danifica uma grande variedade de culturas, vegetais e espécies de árvores, o que acaba por causar enormes perdas econômicas no Paquistão, que é um país agrícola. Prefere viver em terrenos montanhosos, mas comuns em florestas temperadas e tropicais, arbustos e pastagens. Este estudo concentrou-se na identificação e avaliação das lavouras danificadas, juntamente com as principais medidas de precaução utilizadas pelos agricultores locais. Os dados foram coletados de 24 aldeias de dois conselhos sindicais, ou seja, Chamhad e Slahad, do distrito de Abbottabad. Dois tipos de dados (primário e secundário) foram coletados da área de estudo. Foram coletados dados primários para identificação e cálculo de estimativa do total da cultura danificada por meio da observação direta do campo, tomando quadrantes aleatórios em cada aldeia da área de estudo. O dano na cultura foi avaliado pela seleção aleatória de um quadrante de 1x1 m2 para os campos de trigo, milheto pérola e sorgo, enquanto quadrantes de 4x4 m2 foram tomados para milho e legumes. Pelo menos três amostras de quadrante foram colhidas de cada campo, incluindo um quadrante retirado do centro da área do campo. No Conselho Sindical de Chamhad, houve danos ao milho (11,31%) e ao trigo (0,73%) pelo porco-espinho indiano, enquanto no Conselho Sindical de Salhad, danos ao milho (6,95%) e ao trigo (1,6%) foram observados. Em toda a área do estudo, danos gerais à cultura do milho (8,01%) e ao trigo (0,88%) foram calculados. Com base em informações obtidas dos agricultores, o porco-espinho indiano infligiu danos à batata, tomate, couve-flor, pimenta, nabo, rabanete, ervilha, cebola, etc. Dados secundários foram obtidos por meio de um questionário para explorar o conflito suíno humano e as medidas de precaução utilizadas pelos agricultores e proprietários de terras. Questionários abertos e fechados (159) destacaram a presença de porco-espinho indiano na área de estudo e 96% dos entrevistados viram o porco-espinho diretamente. Muitos tipos de medidas de precaução foram utilizados pelos agricultores, como esgrima, estadia noturna, fogo noturno e cães para diminuir os danos na lavoura. Dos entrevistados, 63,91% usaram armas para caçar. No entanto, devido à área ser em grande parte agrícola e ao comportamento noturno da maioria dos porcos-espinhos indianos, 51,57% não usaram nenhuma medida de precaução. O controle biológico do porco-espinho indiano é recomendado na área de estudo. Os agricultores devem ser encorajados e fornecer incentivos, e o uso da corrente para matar os animais deve ser banido, assim como deve ser emitida a licença de caça adequada para superar a caça excessiva. Estudos científicos são necessários para controlar a reprodução de porco-espinho, especificamente nas áreas mais danificadas.

ABSTRACT

Abstract Cranes are the large and attractive Creatures of nature with long necks, legs, and life-span. Adults of both sexes are the same with similar color patterns. Demoiselle cranes spend most of their lifespan on dry grasses. They are also found around the stream, rivers, shallow lakes, natural wetlands, and depressions. To evaluate the current status of habitat use and major threats a study was conducted in tehsil Domel district Bannu. Line transect method and water quality tests (temperature, PH, contamination of E-coli bacteria) were used. To determine the major threats questionnaire method was used. The whole data was analyzed by using SPSS 21 version. Based on the distribution four study sites were selected and four water samples from each study site were taken. Most sites were moderate to highly degraded except Kashoo and kurram river mixing point which was low degraded with livestock grazing and human activities. Water quality tests showed PH ranges from 7-9, temperature 6.5-8.5, and contamination of E-coli in all samples. The social survey revealed that hunting, habitat degradation, and pollution as major threats. Effective long-term conservation and management in the study area are needed to focus on the protection of disturbance-free habitat.


Resumo Guindastes são a grande e atraente criatura da natureza com um pescoço longo, pernas e vida útil. Adultos de ambos os sexos são os mesmos com padrões de cores semelhantes. Guindastes Demoiselle passam a maior parte da vida em gramíneas secas. Também se encontram ao redor do córrego, rios, lagos rasos, pântanos naturais e depressões. Para avaliar o estado atual do uso do hábitat e as principais ameaças, um estudo foi realizado no distrito de Tehsil Domel, em Bannu. Foram utilizados o método transect e testes de qualidade da água (temperatura, pH, contaminação da bactéria E. coli). Para determinar as principais ameaçasfoi utilizado o método de questionário. Todos os dados foram analisados por meio da versão SPSS 21. Com base na distribuição, foram selecionados quatro locais de estudo e quatro amostras de água de cada local de estudo. A maioria dos locais estava moderada a altamente degradada, exceto no ponto de encontro dos rios Kashoo e Kurram, que teve baixa degradação com pastagem de gado e atividades humanas. Os testes de qualidade da água mostraram variação de pH de 7-9, temperatura 6,5-8,5 e contaminação de E. coli em todas as amostras. A pesquisa do questionário revelou que a caça, a degradação do hábitat e a poluição são as principais ameaças. Efetiva conservação e gestão a longo prazo na área de estudo são necessárias para se concentrar na proteção de um hábitat livre de distúrbios.

@#Human brucellosis is a neglected zoonotic problem worldwide with a high degree of morbidity in humans and is mostly overlooked due to other febrile conditions. The aim of this study was to evaluate the sero-prevalence and risk factors of human brucellosis among subjects living in Punjab, Pakistan. In this cross-sectional study, human blood samples were collected from seven districts of Punjab, Pakistan. Information regarding personal data, demographic data and potential risk factors was collected through a structured questionnaire. Detection of anti-Brucella antibodies was done through Rose Bengal Plate Test (RBPT) and Enzyme Linked Immunosorbent Assay (ELISA). Descriptive analysis, Chi square test and Odds ratio was applied using STATA software version 12. The sero-prevalence of human brucellosis was 13.13% with significantly higher percentage in males 17.23% and age group 25-40 years 16.50% (P=< 0.001). The demographic factors positively associated with human brucellosis were lack of education (P = 0.003; OR = 1.85) and farming as an occupation (P =<0.001; OR = 2.50) Similarly, among the risk factors studied, keeping animals at home (P =<0.001; OR = 2.03), slaughtering of animals (P =<0.001; OR = 15.87) and consuming raw milk (P =<0.001; OR = 5.42) were the factors strongly connected with human brucellosis. A massive awareness should be given to livestock farmers and individuals directly linked to animals regarding risk factors and transmission of brucellosis. Consumption of unpasteurized milk and its products should be condemned to curtail this neglected disease.

@#Toxoplasma gondii is a protozoan parasite that can infect all mammals, serving as intermediate hosts. The cause of congenital toxoplasmosis is transplacental transmission of the parasite to the foetus, resulting in wide range of manifestations from mild chorioretinitis to miscarriage. Its frequency can be reduced by early screening of pregnant women which is based mainly on tests for anti-Toxoplasma antibodies. We collected serum samples of 594 pregnant women (subjects) after taking their consent over a period of two years (2016-2018) and analyzed them for anti-Toxoplasma IgG by ELISA. The positive samples were then analyzed for IgG avidity test which could differentiate between recent and past infections. The seroprevalence was also correlated with the age of the subjects and their contact with cats. 162 subjects were found positive out of which only three showed a recent infection. After following up until delivery, one of them delivered a baby who had jaundice and was diagnosed with anti-Toxoplasma IgM at birth. The foetus of the second subject died in-utero, while the third woman delivered a normal baby after being given spiramycin when diagnosed with toxoplasmosis in the first trimester. It was found that most of the positive subjects had frequent contact with cats. Invasion of the parasite during third trimester resulted in death in-utero and jaundice. Most common cause of pregnancy wastage during our study was spontaneous abortions while pregnancy loss due to congenital anomalies was rare.

The popliteal artery is the branches off from the femoral artery. It is located in the knee towards the back of the legs. The clinicians and radiologists dealing with the popliteal area use description of branching pattern of the popliteal artery, in which it first gives rise to anterior tibial artery followed by the tibial peroneal trunk, which then terminates by dividing into the posterior tibial and peroneal arteries. As popliteal artery is one of the most common sites for aneurysm formation, the knowledge of the normal popliteal artery diameter is essential to determine the relevance of popliteal dilatations. Objective: Considering the clinical importance of diameter of popliteal artery, this study was undertaken to note the internal diameter of popliteal artery. Subjects and Methods: The present study was conducted on 50 specimen embedded in 10% formalin from the department of Anatomy of tertiary care hospital & medical college. Cadavers which had gross pathological deformities in its lower limbs were excluded from the study. All the cadavers were adult males and females. Results: This study states right popliteal arteries are slightly larger (4.9 ± 0.7 mm) as compare to left popliteal artery (4.8± 0.5 mm). This study also reconfirm popliteal arteries of men are slightly larger than that of female (4.9 ± 0.7 mm), (4.6± 0.9 mm) and left popliteal artery (4.8± 0.5 mm), (4.6± 0.7 mm) respectively. On the other hand terminal part its diameter is slightly larger in male (4.5 ± 0.6 mm) as compare to female (4.3 ± 0.5 mm). This study also shows terminal end of left popliteal arteries (3.8 ± 0.7 mm) are smaller than right popliteal arteries (4.3 ± 0.5 mm). Conclusion: The present study was done in a small cadaveric sample. In our prospective different live and cadaveric study will be helpful for vascular surgeons, radiologists. The variation in the measurement of our study compare to other study may be due to the difference in live and cadaveric spacemen or due to the method of measurement. Therefore, we suggest before making any decision study on much larger sample should be consider

ABSTRACT

Background: Differentiation of hepatocellular carcinoma (HCC) from metastatic malignancy in liver may be difficult at times on fine-needle aspiration cytology, especially in case of moderate-to-poorly differentiated tumors. The benefit of cell-block technique is the recognition of histologic pattern of diseases along with application of a wide variety of immunohistochemical (IHC) stains to differentiate hepatic malignancies. In this study, CD10 IHC staining was done on cellblocks prepared from aspirates of clinicoradiologically/cytologically suspected malignant liver neoplasms to differentiate HCC from malignancies metastasizing to liver. Objective: The objective of the study was to assess the diagnostic utility of CD10 IHC stain on cell-block preparation for differentiating primary from Secondary malignancies of liver. Materials and Methods: Formalin-fixed, paraffin-embedded cellblocks of 61 cases (25 cases of HCC and 36 cases of metastatic carcinoma) were prepared from a fine-needle aspirate of the suspected malignant liver neoplasm and immunostained using monoclonal antibody against CD10. Results: Twenty-two (88%) of 25 cases of HCC were positive for CD10 with a canalicular staining pattern. Two (8%) were positive for CD10 with membranous and one (4%) with cytoplasmic staining pattern. Conclusion: CD10 immunostaining on cellblock is useful in discriminating HCC and metastatic carcinoma of the liver with a diagnostic accuracy of 88.52%.

Background: Worldwide primary angioplasty is a recommended strategy of reperfusion in patient with acute myocardial infarction as because it ensures reperfusion of the infarct-related vessels more than 90% where as, with thrombolytics it is only 60-70%. Methods: It is a retrospective observational study includes all patients treated with primary angioplasty at United Hospital from Between March 2007 to August 2010. Total 237 consecutive patients with acute myocardial infarction were treated with primary angioplasty were included. Those presented beyond 12 hours of onset of chest pain, in cardiogenic shock, resuscitate and intubated before the procedural were excluded from the study. Results: Majority (76%) of the patient were male, age was minimum 28 years and maximum 80 years, 41.5% were diabetics, 58.4% were hypertensive, 43.5 %were dyslipidaemic, 17% were smoker, 29.3% with positive family history. Fifty seven percent patients presented with anterior MI, 42 % with inferior MI and 1 % with lateral MI. Left anterior descending (LAD) is the most common vessel involved (S7%), followed by Right coronary artery (RCA) 31 %, Left circumflex artery (LCD 8 %, Ramus 1.3 % and Graft vessel 2.7%. Our door to balloon time was minimum 23 min, maximum 184 min. We used drug eluting stents for most of the patients , GP IIbIIIa receptor Mockers used in 50% cases and thrombus suction device were used when indicated. We faced complications like arrhythmias in 24% hypotension in18 %, no flow or slow flow in 45%, cardiac arrest in 3% and coronary perforation in 1. %. Our overall survival was 97.9 %. Conclusion: Primary angioplasty is a emerging area in the context 149-154of our country . Many of the new centers start this novel strategy which helps to save many lives. Primary angioplasty is feasible and safe method of reperfusion in patient with acute myocardial infarction in our center.

Background: Carotid angioplasty & stenting is becoming an emerging therapeutic option for carotid revascularization. The use of cerebral protection system has expanded the area of application of the procedure worldwide. Purpose: To assess the feasibility, success rate, safety as well as in-hospital & early 30 days outcome in patients undergoing percutaneous carotid intervention. Methods: A retrospective, observational study where a total of 18 (Eighteen) consecutive patients who presented with symptomatic and > 70 % carotid artery stenosis & asymptomatic but > 90% stenosis underwent percutaneous carotid intervention. All of them had coronary artery disease; CABG was done in 3 patients & PCI in 9 patients. Three of them had previous stroke (Ischemic) & 7 had TIA. Results: Technical and angiographic success was achieved in all patients. Carotid artery obstruction diminishes from 85 + 14 % to 10 + 5 % (p< .001). Mean lesion length was 12 + 3 mm and mean time of carotid occlusion during balloon inflation was 10 + 2.5 sec. distal protection devices used in all patents. No major stroke or death occurred during procedure. One patient developed No-flow because of obstruction of distal protection device which was managed by thrombosuction. One patient developed TIA. All patients were discharged from hospital after an average of 3 days & all of them were prescribed dual antiplatalet therapy for 6 months. During follow-up one patient died secondary to acute myocardial infarction and one patient developed major stroke. Conclusion: Percutaneous angioplasty and stenting associated with distal protective devices appear feasible, effective and almost safe endovascular treatment modality for carotid artery stenosis.

It has been postulated that mushroom has beneficial effect of lowering blood glucose and cholesterol in diabetic subjects. The literature so far searched and found that there was no published data in this regard. This study was undertaken to assess the effect of reducing blood glucose, cholesterol and triglycerides in diabetic patients. Additionally, this study addressed whether there was any hepatic and renal toxicity of mushroom. This clinical investigation was conducted in BIRDEM hospital from July 2005 to January 2006. Eighty-nine subjects were recruited. Baseline investigations included height, weight, blood pressure (SBP, DBP), plasma glucose for fasting (FPG) and 2-h after-breakfast (2hPG), total cholesterol (T-chol), triglycerides (TG) and high-density lipoprotein (HDL-c). Twenty- four days' study constitutes 7-days mushroom, 7-days no mushroom and then 7-days mushroom. Investigations were done at the start and each after every 7-days. Thirty subjects (M / F = 17 / 13) followed to ensure full compliance with the designed protocol for 24 days. The mean (SD) age of the participants was 46.3 (10) years. Mushroom significantly reduced systolic and diastolic blood pressure (SBP, p<0.01; DBP, p<0.05). It also lowered both plasma glucose significantly (FPG & 2-hPG, p<0.001). Mushroom also lowered total cholesterol and TG significantly; whereas, there was no significant change in weight and HDL-c. When mushroom was withdrawn, there were significant increases of DBP, FPG, 2hPG, T-cholesterol and TG, whereas, no significant change was observed in weight, SBP and HDL-c. Restarting mushroom there was again significant reduction of blood glucose, TG and cholesterol. We conclude that mushroom significantly reduced blood glucose, blood pressure, TG and cholesterol of diabetic subjects without any deleterious effect on liver and kidney. The effect of mushroom may be investigated in a large sample for a longer duration to evaluate its efficacy and toxicity.

ABSTRACT

The aim of this study was to determine the critical incidents that contribute to the initiation of substance use and abuse among women in Trinidad and Tobago. Twenty women were randomly selected from 46 women currently attending 43 drug rehabilitation centres, Narcotics Anonymous and Alcoholics Anonymous groups in Trinidad and Tobago. In-depth semi-structured interviews using the critical incident technique were conducted. Interviews were recorded, transcribed and analyzed. Concepts, categories and themes were determined by team study and group discussion. The critical incidents that influenced women to initiate the use and abuse of substances fell into eight major themes: factors intrinsic to the individual woman, family factors, social and environmental factors, life stresses, relationship issues, abuse, peer pressure and substance use and abuse as a coping mechanism. The results imply that the factors contributing to the initiation of substance use and abuse among women in Trinidad and Tobago are many and complex. As such any attempt to address this issue requires a broad-based approach. Such an approach should address family use of such substances, societal acceptance of them, availability, the self-esteem of the individual woman and her ability to cope with peer and internal stresses


El objetivo de este estudio era determinar los incidentes críticos que contribuyen a la iniciación en el uso yabuso de substancias entre las mujeres en Trinidad y Tobago. Se seleccionaron veinte mujeres al azar, de46 mujeres que asistían a 43 centros de rehabilitación de drogas, grupos de Alcohólicos Anónimos, y Narcóticos Anónimos en Trinidad y Tobago. Se realizaron entrevistas profundas semi- structuradas usando la técnica de incidentes críticos. Las entrevistas fueron grabadas, transcriptas, y analizadas. Los conceptos, categorías y temas fueron determinados mediante estudio en equipo y discusión en grupos. Los incidentes críticos que influyeron en que las mujeres se iniciaran en el uso y abuso de substancias comprendían ocho temas principales: factores intrínsecos a la mujer como individuo, factores familiares, factores sociales y medioambientales, estreses cotidianos, problemas en las relaciones, abusos, influencia de los amigos y malas compañías, y el uso y abuso de sustancias como mecanismo para hacer frente al estrés. Los resultados implican que los factores que contribuyen a la iniciación del uso y abuso de sustancias entre las mujeres de Trinidad y Tobago son muchos y complejos. Siendo así, cualquier intento por abordar este problema requiere un enfoque amplio. Tal enfoque debe abordar el uso de substancias por parte de la familia, la aceptación social de las substancias, la disponibilidad, la autoestima de la mujer como individuo y su capacidad para hacer frente tanto a su estrés interno como al que proviene de la presión que ejercen amigos y malas compañías.
